Worked example
Statement: "Preceptors are selected for teaching ability, not just seniority or availability."
A manager assigns whoever is on the schedule. Strong nurses, no preceptor preparation, no selection criteria. That is Rarely (1), not Sometimes (2). Sometimes means criteria exist and get applied unevenly. Rarely means the criteria do not exist.
Scoring generously here is the most common way this tool gets misused. The score is only useful if it hurts a little.
