The scoring scale and assessment principles below are grounded in established nursing professional development frameworks. The tiered remediation structure and its timelines are a practical design consistent with those frameworks; adapt them to your organization's competency and remediation policy.

A note on the tiers. The two-tier remediation pathway and its suggested timelines (for example, coaching within 3–5 shifts, re-check within 1–2 weeks) are a practical structure designed to be consistent with the competency-based orientation principles above. They are not prescribed by any single published protocol. Treat them as an adaptable starting point and align them with your own competency and remediation policy.
